By agreeing to this form, I realize that the training and exercise of doges sometimes involve vigorous physical activity and possible injury. I agree to accept the risk involved in such activity or injury and hereby agree to assume all liability for any personal injury, property damage, and injury or damage, and injury or damage to other dogs that may be caused by the dog I bring to class. I absolve Four Paws Rehabilitation, its instructors, and staff from such liability. I further represent that the vaccinations indicated have been administered to the dog named and that same are current.

Vaccination Requirements(Required)

I agree to the following:

The required vaccination are rabies, distemper, hepatitis, parvo, and parainfluenza, and they must be given two weeks before class starts. All dogs/puppies are to be vaccinated (including rabies) following their veterinarian's protocol.

Payment and Shot Record(Required)

I agree to bring the vaccination record the first night of class.

I agree to bring cash, check or card to pay in full the first night.
